File a Claim
The Administrative Services Department processes personal injury, property or vehicle damage claims filed against the City of Duarte. The terms or acceptance of claim forms by the City is not an admission of any fault or liability on the part of the City or any office, agent, or employee therof.
To file a claim against the City, complete the General Claim form. The Claim Form can be downloaded, emailed or mailed to you. Forms are also available on a walk-in-basis. The completed form must be returned in person or by mail.
As with all claims, please make sure to provide any information to support your claim (i.e. estimates, photographs, diagrams, etc.)

File an ADA Complaint
The City of Duarte wants to hear concerns and complaints from citizens in order to provide accessible programs, services, and activities. A member of the public can contact the City of Duarte with a comment, concern, or complaint without filing a formal grievance. Contact the Administrative Services Department at (626) 357-7931, ext. 211 for ADA related comments, concerns or complaints.
A formal grievance can be filed by completing the City of Duarte Grievance Form no later than 60 days after the alleged violation. You can file an American with Disabilities Act complaint by mail or via email.
